MINOR WAIVER OF LIABILITY CPR class 2022

  CPR CLASSES are being offered the third Wednesday of each month, beginning at 6 p.m. and lasting approximately three (3) hours. A registration form must be completed and confirmed by the District prior to attending the class. Class size is limited to 15 registrants. District residents receive preference, and there is no fee for District residents.  There is a $25 per person fee for non-District residents. Forms must be emailed to [email protected] OR faxed to: 636-272-7857, OR mailed or dropped off with $25 Fee (if required) to: 111 Laura K Dr., 2nd Floor, Attn: Julie Gaylord, O’Fallon, MO, 63366.
